Why would I choose Pediatric Plus?
The providers are highly qualified with years of experience. Parents will actively participate in care decisions. Anything that makes sense and is in the best interest of the child, will be done. All blood work and vaccinations are done in the office. We have a large and strong referral base to various specialists. We are open Weekdays till 7 PM. We are open all SATURDAYS. We are ON CALL 24/7. Our office is child friendly. We have an extraordinary staff. Most patients come to us because of Word Of Mouth. We will be honored by your trust.
Are you easily accessible?
Yes.
We are located at the corner of STEINWAY & BROADWAY, on 3rd Floor, in an Elevator Building. We are half a block from the Subway.
We see sick patients on the same day, during office hours.
The Doctors are accessible to you as you would expect from high quality providers.
How do I make an appointment?
Please call our Office at 718-626-4881 and make an appointment. If your child is sick, we will try to see you on the same day.
Does Pediatric Plus accept my insurance?
Review your plan directory or contact your insurance company to make sure INDRANIL GUPTA, MD (Physician-in-charge at Pediatric Plus) participates with your insurance. You can always call us at 718-626-4881 for more information. We accept almost all Insurances.
How do I enroll my newborn in my plan?
Most insurance plans will give you 30 days to enroll your baby. Please check with your plan immediately after the baby is born. Under many plans, child is normally seen under parent’s insurance plan, for first 30 days.
What do I bring to my first visit?
First, please plan to arrive earlier than your appointment time.
Please bring your child’s Insurance Card / Immunization Record (If you have one).
Please make sure you have changed Primary Care Physician ( If your plan requires it ) to INDRANIL GUPTA, MD (If you need help, give us a call).
You will need to fill out some forms when you arrive. You can print these forms and fill them out before you arrive. Just click on the Patient Forms.
How do I transfer my child's medical records?
Please go to Patient Forms section and fill out the appropriate form and send to your previous doctor. You may choose to do so when you come to our office. Just ask the receptionist.
Do you separate newborns from other patients?
Yes, we will.
How do we make follow up appointments?
Before you leave the office, make sure you make a follow up appointment at the reception desk. You may choose to call us with appointment request.
